Health Assessment

Registered Nurses, highly experienced in clinical assessment complete assessments in conjunction with your own doctor.

A Registered Nurse undertakes these assessments in your  home, where a thorough safety check is also undertaken. Home visits provide the chance to check the safety of wet areas, kitchens, steps, stairs, floor safety and access to outside of the building. This information is vital in making an accurate risk assessment for falls.

The assessment is an opportunity for clients to discuss any health issues of concern to them. Our nurses can assist with issues such as incontinence, diabetic/dietary advice, hearing, dental, eyesight, physical limitations, social wellbeing, transport, cleaning and general health issues. Your nurse will thoroughly review any medications taken to ensure correct compliance and storage.

With the client’s consent a physical examination can also be undertaken. This assessment can include: pulse, blood pressure, height, weight, vision, hearing, urine test and foot check.
